Complete the Short Loan Application if you wish to pre-qualify online. This document does not include sensitive information that you would not want a third party to read. The Click n' Print method is designed to be printed, completed and FAXED back.

The complete loan application package is described in items 1-3 below. To ensure timely closing of your loan, Mid-Cities Mortgage Corporation recommends that you contact your settlement agent within five (5) business days after submitting your loan application.

1. Temporary Loan Registration/Lock-In Sheet - Also known as the Lock-In Sheet, this form can be submitted by fax or delivered in person. Check our rates page or a Mid-Cities Mortgage associate for the most current rates.

All required application and processing fees must be paid upon submission of the Lock-In Sheet. Checks and credit cards are accepted for payment of fees. Be sure to complete the credit card authorization section if you are filing by fax.

Mid-Cities Mortgage Corporation Application and Processing Fees are non-refundable and are assessed as follows:

Application Fees:
  • $ for non-investment, single-unit properties for a single borrower or borrower set,
    valued at less than $.
  • $ CALL for properties valued at greater than $.
  • $ for loans with multiple co-mortgagors or co-mortgagor sets.
  • $ for single-family investment properties.
  • $ for 2-4 unit properties.

    Processing Fee: $
    The Processing Fee covers charges for document review, tax services, underwriting, and flood certification, and is credited back to the borrower(s) at Closing.

    Final Inspection Fee: $
    Applies only to new construction loans.


2. Borrower's Certification and Authorization This form, which is used to obtain your credit report and credit and income verifications, must be signed by all borrowers before we can begin processing your loan.

3. Required supporting credit documentation To ensure lock-in of your interest rate, Mid-Cities Mortgage Corporation must receive these items within two (2) business days of filing your initial lock-in. Please see page 3 for a list of required items.

Mid-Cities Mortgage Corporation charges no loan origination fees, nor is there any charge for the credit report or home appraisal. In addition to the Application, Processing, and Final Inspection fees described in item 2 above, the only fees paid to or collected by Mid-Cities Mortgage Corporation are any points paid to discount rate, and mortgage insurance charges where applicable.

As part of our service, within three (3) days after we receive your completed loan application, Mid-Cities Mortgage Corporation will prepare a rough estimate of other taxes and fees which will apply to your loan. These other taxes and fees will be collected by your title company and local municipal authorities. Contact the settlement company that will be handling your loan closing for the most accurate estimate of these fees.
